This HIV Special Needs Plan Model Contract is presented for informational purposes only. It represents the HIV SNP Model Contract language effective January 1, 2007 as approved by the Centers for Medicare and Medicaid Services (CMS) and may not reflect minor amendments (if any) approved subsequent to January 2007.

This model contract also may not contain some local district-specific provisions that may be included in a SNP's contract with a locality.
