Forum Registration Process

If you wish to attend this forum, register in advance on-line at http://www.zoomerang.com/Survey/?p=WEB227LBETLDB8 or by calling the Alzheimer's Association at (800) 272-3900 or the Erie County Department of Senior Services at (716) 858-8526. P lease indicate when registering whether you would like to speak at the forum. Speakers will be limited to five minutes in order to accommodate all individuals wishing to speak.

Community members are invited to submit written comments at the forum, e-mail them to forums2008@health.state.ny.us or mail them to Mary Ann Albano, NYSDOH, 150 Broadway, Room 350, Albany, NY 12204.

General and Specific Driving Directions:

From the NYS Thruway, take exit 52 Walden Ave East. Merge onto Walden Ave. From Walden Avenue, turn right onto Union Road/NY 277 South. Pass under 2 railroad bridges. Just after the second bridge, turn right onto an access road that takes you to Broadway. Turn right at the traffic signal onto Broadway. Go straight through the next traffic light. The Cheektowaga Senior Citizen Center is the third building past the Town Hall on the right. It is a single floor brown brick building. Parking at the Senior Citizen Center is free.
