Forum Registration Process

If you wish to attend this forum, please register in advance on-line at http://www.zoomerang.com/Survey/?p=WEB227LBETLDB8 or by calling the Alzheimer's Association at (800) 272-3900 or the Monroe County Department of Human & Health Services, Office for the Aging at (585) 753-6280. Please indicate when registering whether you would like to speak at the forum. Speakers will be limited to five minutes in order to accommodate all individuals wishing to speak.

How to Submit Written Comments

Community members are invited to submit written comments at the forum, e-mail them to forums2008@health.state.ny.us or mail them to Mary Ann Albano, NYSDOH, 150 Broadway, Room 350, Albany, NY 12204.

Specific Driving Directions to the R. Thomas Flynn Campus Center, Building 3

From East Henrietta Road: Take a left or right onto Campus Drive and continue driving to the back of the campus. Continue until you reach Center Road. Follow the signs to parking lots M and M1. You may park in either parking lot M or M1.

From Brighton-Henrietta Townline Road: Turn right onto Campus Drive and continue driving to the back of the campus. Continue until you reach Center Road. Follow the signs to parking lots M and M1. You may park in either parking lot M or M1.

After parking, enter the R. Thomas Flynn Campus Center, Building 3, through the North Entrance (closest to the pond). Enter the Bausch-n-Lomb lobby. The Forum Room is straight ahead.
