SUBJECT: Record of Appearance - New Form
The newly-created New York Commission on Public Integrity (the Commission) will continue to perform the work that has been done by the New York Temporary State Commission on Lobbying and the New York State Ethics Commission. The Commission has requested that all state agencies begin using their Notice of Appearance form. This form replaces the Attendance Log the Department of Health has been using.
