"Translating Data into Action: The Impact of Community Health Assessments in New York States" summarizes findings from a survey of 149 assessment practitioners, interviews with eight of them, and follow-up discussion with about 70 practitioners at a conference. The survey responses identify three major contributing factors that influence the process or success of community assessments: leadership, quality of collaboration; and staff dedicated to the community assessment process. The report identifies immediate and short-term public health system impact of community health assessments, and recommends a more in-depth study to understand the long-term impact.
