Definition:

The number composed of the Department of Health's four-digit Permanent Facility Identifier (PFI) followed by a check digit (for SPARCS control purposes).

Department regulations state that services must be reported under the physical location where they are provided. Common ownership of different facilities does not change this requirement.

Codes and Values:

  1. A five-digit number.

  2. Must be a valid number in accordance with the SPARCS Hospital Profile Reference File maintained by the SPARCS Administrative Unit.

Edit Applications:

  1. Must be a valid entry.

  2. Files submitted via the Health Provider Network (HPN) must have data for only one PFI.
